Client-Administered Medication Management Initiative: Design, Implement, Hand Off

When a large state Medicaid agency identified unusually high utilization of health resources among asthma patients in a certain geographic area, Clinical Pharmacy Services was asked to develop a pilot medication therapy management program that could be implemented by community pharmacists.

Our team set up and launched the program, which was initially limited to adult patients who were covered by Medicaid and had a diagnosis of asthma:

• Designed framework
• Determined how to train pharmacists for face-to-face medication therapy management sessions to address asthma and other chronic diseases
• Recruited and trained pharmacists
• Identified decision-support software
• Developed standardized recordkeeping system for pen-and-paper interactions
• Recruited patients and assigned them to community pharmacies
• Addressed language issues

Shortly after the program’s launch, our client took over administration responsibilities as planned.
